Who it is for

  • Labour hire companies applying for a first policy or renewing.
  • Firms asked by QBE for the Labour Hire Accountant's Declaration, form QM10139, or by another insurer for the same thing.
  • Companies that have not started trading yet.

What you get

  • The declaration signed. Where the company has not traded, a scoped declaration stating what was checked, because the printed sentence cannot honestly be signed about wages not yet paid.
  • Each host's classification determined, and your wages split across them by placement dates.
  • The remuneration base checked: allowances, salary sacrificed super and fringe benefits in, compulsory super out.
  • A placement register set up to the WorkCover WA Appendix 1 template.

What we need from you

  • Hourly rates, roster and allowances for each role.
  • Start date and policy period.
  • Each host's name, contracting entity and, if you have it, their own classification.
  • Whether superannuation sits on top of the rates or inside them.
